completed with each participant during intake. These are tools for assessing the participant's ability to read, write, speak, and comprehend English.
They will also enable tutors to center instruction around the goals of the
learner.
Information to be collected by tutors
Student Profile Sheets to be completed for each student and shared with
coordinator at the end of each term.

Attendance Sheets for Tutors to be completed and shared with the coordinator
at the end of each term.Tutor
Timesheets to track tutor preparation time and time tutoring. Please
share with tutor coordinator.
Data Collection Spreadsheets for Coordinators
Program data sheets should be completed
by Coordinators and shared with Oregon Literacy at the end of each term.
Please use the Student Attendance Sheets and Tutor Timesheets to compile the information.
Note: there is a worksheet for Students and a worksheet for Tutors.
