Tutor Packet
Tutors will complete a packet for each student during the term. The packet consists of a Learner Needs Assessment, which will aid all ESOL learners in setting goals for instruction; tutor timesheets; and learner timesheets, which will assist coordinators in collecting valuable information about their programs.

Evaluation Surveys
Access the evaluation surveys here. Learners will be asked to complete a survey at the end of each term. Each tutor will be asked to complete at least one survey each year. Program coordinators may ask tutors to complete a survey at the end of each term.

Data Collection Spreadsheets for Coordinators
Program data sheets should be completed by Coordinators and shared with Oregon Literacy at the end of each term. Please use the Student Attendance Sheets and Tutor Timesheets to compile the information. Note: there is a worksheet for Students and a worksheet for Tutors.
